Type: The format of the material (Book, DVD, Presentation, Event, Other).
Received: The date in which the request for reconsideration form was received by the Library.
Reviewed: The date in which the request for reconsideration was evaluated by the Board of Trustees.
Decision: The Board of Trustees decision to retain or remove the material from the library's collection.
Reason: A brief reason for the decision, or link to a public response to the reconsideration request.
